Welcome to my new EMR website! In my attempt to move to electronic documentation instead of paper charts, I have instituted the use of Electronic Medical Records (EMR). This website is HIPAA complaint and has security safeguards in place. A prospective client as well as current clients can securely access a portion of their record to complete and electronically sign required admission paperwork, including the Notice of Privacy Practices, Financial Agreement, Requests for Release of Information, and clinical assessments. After your initial call to my office, I will give you an unique USER ID and PASSWORD that will enable you to securely log into the “Client Login”.
Once logged into the EMR, you will be taken to the HOME screen where you will see a box that says “Alerts”. Click on the link of each form, then read and complete the form. After you have read and completed each form, enter your password in the box labeled “password” at the bottom of the form and then click on the icon “All Done? Enter Your Password and Click Here”. To return to the “Alerts” page, click on the <<back button.
